Empowering Lives After Injury
Case Management is available to all service recipients living in the community. We follow a team approach to programming and work collaboratively with our clients, caregivers, clinicians and professional consultants to address the individual’s unique needs in all areas of independence and community integration.
Case Managers support clients in the following areas:
- Budgeting and money management;
- Physical health and medication management;
- Organization and time management;
- Instrumental activities of daily living, such as grocery shopping, meal planning and meal preparation;
- Academic and vocational assessment and placement;
- Transportation training and community navigation;
- Self-advocacy and self-awareness;
- Liaise and connect with external community agencies to coordinate resources
- Annual service plan reviews to assess and plan for future needs

We partner with local organizations across Oakville, Orangeville, and Brampton, which generously provide their facilities and equipment and allow us to reach clients who need access to our head office due to transportation barriers.
These partnerships are mutually beneficial as they enable us to spread awareness about brain injury within the broader community.
