Supported Independent Living

The Supported Independent Living (SIL) program was launched in 2012 with a focus on providing community-based rehabilitation and support to individuals living independently in the community who do not require the same level of support offered in ABI specialized assisted living, long term care, or group home settings.

Clients accepted into the program participate in an Independent Living Assessment which evaluates a variety of domains such as: cognitive, memory, attention, language, social, ADLs/IADLs, etc. The results of this assessment help to identify the specific rehabilitation needs of each individual, which are then taken into consideration while developing their rehabilitation program.  

The SIL program operates seven days a week, and is facilitated by five front line staff and two case managers. Our team has received a variety of specialized training focused on ABI rehabilitation, mental health and addictions.   

Team members provide 1:1 in-home support to assist clients in achieving their personal and rehabilitation goals including medication administration, transportation training, attending community-based appointments, budgeting, grocery shopping, cooking, and housekeeping.  Case managers also oversee goals such as securing financial, housing, legal, medical, and a variety of other supports a client may require.

The SIL program works in collaboration with many community partners such as the Region of Peel, Supportive Housing in Peel (SHIP), and Able Living, which allow us to provide subsidized housing to suitable clients.
